Why new runners get hurt

Novice runners are injured at more than twice the rate of experienced ones. The reason is not your body.

The number that reframes this

A meta-analysis pooling injury rates across running populations found 17.8 injuries per 1000 hours of running in novice runners, against 7.7 per 1000 hours in recreational runners. [1]

More than double, for the same activity. The difference is not that beginners have worse bodies. It is that beginners are in the part of the curve where tissue has not yet adapted to the load, and where the load is usually being added fastest.

Which is oddly good news. A risk that comes from how you start is a risk you can do something about. A risk that came from your age or your build would not be.

Shin splints, specifically

Medial tibial stress syndrome, which nearly everyone calls shin splints, is the classic first injury. A systematic review of the condition in novice and recreational runners puts incidence in runners at roughly 13.6 to 20 percent, with the higher rates among novices. [2]

It presents as a diffuse ache along the inner shin that shows up during or after running and eases with rest, at least at first. The review points at sudden rises in training volume, intensity and impact as the modifiable contributors, alongside a set of biomechanical factors you mostly cannot change. [2]

The distinction worth drawing is between a diffuse ache along a stretch of shin and a sharp, precise, one-fingertip point of bone pain. The second one is worth getting looked at rather than managed.

It is the ramp, not the runner

Researchers followed 110 people, average age 47, through beginner running programs in the UK. 27.3 percent finished. Of those who dropped out, 73 percent reported a musculoskeletal injury, and when the reasons people gave were grouped, one of the three themes was how fast the program progressed. [3]

That is a design failure being paid for by the participants. A plan built on a calendar tells you to run three minutes in week three whether or not week two left you limping, because a calendar cannot see you.

The usual advice at this point is to add no more than ten percent a week. It is a reasonable instinct and it has never held up well in trials, largely because a percentage of a small number is still arbitrary and still blind to how the last session actually went.

What a slower ramp actually looks like

Start with walking, for longer than feels necessary. Three twenty-minute walks is a real training week when you have not done anything in years.

Let running in gradually and in pieces, and keep nearly all of it easy enough to talk through. Going harder than that is the thing that generates the load you have not adapted to yet.

Gate the progression on something that can see you. A heart-rate ceiling responds to a bad night, a hot day, or a week that took more out of you than it should have. A calendar does not.
