The science behind running
What running does to your heart, your muscle, and how fast both of them age.
Your aerobic fitness predicts how long you live
Cardiorespiratory fitness is how much oxygen your body can take in and put to work when you are going hard. The ceiling is called VO2 max, and it is measured in milliliters of oxygen per kilogram of body weight per minute.
It is also one of the strongest predictors of mortality anyone has measured. One study followed 122,007 adults who had taken a treadmill test, with a median follow-up of 8.4 years. Fitness was inversely associated with all-cause mortality, and the benefit had no observed upper limit: the fittest group still did better than the group below it. [2]
The comparison that lands hardest is this one. In that data, the added mortality risk that came with low fitness was comparable to or greater than the risk that came with coronary artery disease, smoking, or diabetes. [2]
A separate meta-analysis put a number on each step up. One MET, roughly 3.5 milliliters of oxygen per kilogram per minute, was associated with about 13 percent lower all-cause mortality and about 15 percent lower risk of cardiovascular events. [3]
You do not need an athlete's number. You need to move up from wherever you are standing, and the steepest part of that curve is at the bottom, which is exactly where you are if you have not run in years.
Aging takes muscle. Training takes some of it back.
After about age 30, adults lose somewhere between 3 and 8 percent of their skeletal muscle per decade, and the rate picks up later in life. [5] That loss is usually why stairs get harder long before anything actually hurts.
In 2011, researchers imaged the thighs of 40 masters athletes aged 40 to 81 who trained four to five times a week. Mid-thigh muscle area, quadriceps area, and lean mass did not decline with age across that entire forty-year span, even though total body fat percentage did rise. [1]
The figure from that paper that gets shared most is a row of mid-thigh cross-sections: a 40-year-old triathlete, a sedentary 74-year-old man, and a 70-year-old triathlete. The two athletes look like each other, thirty years apart. The sedentary man, four years older than one of them, does not. Less muscle, and fat distributed through what is left.
The authors put the conclusion plainly: the usual decline in muscle mass and strength may signal the effect of chronic disuse rather than the effect of aging by itself. [1] Training did not stop these people getting older. It changed what getting older did to the tissue that carries them around.
Bone follows the same logic, which is why the drawing above hollows out rather than simply shrinking. Cortical bone thins from the inside as the marrow cavity widens, so what is lost is wall thickness rather than overall width. A study scanning the tibias of masters sprinters, distance runners, race-walkers and sedentary controls found bone mass and geometry tracking how hard each discipline loaded the leg, with the sedentary group at the bottom of the range. How fast fitness fades depends on what you keep doing
VO2 max falls with age in everybody. How fast is not fixed, and it is not mostly about age.
A 2022 review of longitudinal studies in masters endurance athletes found declines ranging from 5 percent to 46 percent per decade. That is not a typo, and the spread is the finding. Changes in training volume explained 54 percent of the variance in men and 39 percent in women. [4]
Read that the other way around. The athletes who held their training volume sat at the shallow end of that range. The ones who cut back fell faster, and the ones who stopped fell fastest of all. Age set the floor. Behavior set the slope.
Why heart rate is the input worth controlling
All of the above argues for aerobic training. None of it tells you how hard to go on a Tuesday evening in week two, and that gap is where most beginners come apart.
Pace does not know anything about you. A ten-minute mile is a warm-up for one person and a maximal effort for another, and it is a different effort for the same person on a hot day, after a bad night, or at the end of a heavy week.
Heart rate does know. It is a direct readout of what the work is costing you right now, which makes it the one input that adjusts itself when you are tired, when you are unwell, and when you are fitter than you were last month.

Common questions
Is it too late to start running in your 50s or 60s?
The research on masters athletes runs to age 81, and the fitness benefit in the large mortality studies held across age groups. Starting later means starting from a lower base, which is where the returns per unit of effort are largest, not smallest.
What is VO2 max and why does it matter?
VO2 max is the most oxygen your body can use per minute when working hard, scaled to your body weight. It matters because it is one of the strongest predictors of mortality that has been measured, and unlike your age it responds to training.
Does running actually preserve muscle as you age?
Aerobic training is associated with preserving muscle, though strength training is the more direct tool for it. In the masters athlete study, mid-thigh muscle area did not decline between ages 40 and 81 in people training four to five times a week.
